This form is recommended for efficiency, however, please feel free to fax, email, or call with pertinent information.

Please complete the following information for "report type". These terms are governed by the Uniform Standards of Professional Appraisal Practice. If you desire a fee quote based on multiple scenarios, please discuss in the Comments/Additional Information section below.

Report Type:

A "self-contained" report is the most thoroughly explained document used by clients. Most appropriate for use in court.

A "summary" report summarizes the data and is the second most thoroughly explained document used by lenders. Most appropriate for use by lenders.

A "restricted" report merely states the conclusion. Recommended for use by clients only. Most appropriate for use by property owner who does not intend to provide a copy to a third party.
